Checklist for borrowers (or download in PDF format)

Salaried/Hourly

1. Last two years of tax returns and W-2s
2. Two most recent pay stubs
3. Last two month's bank statements
4. 1099s or award letters for Retirement, Social Security or Disability benefits income, if applicable

Self-Employed

1. Last two years of tax returns or last two years of bank statements (personal or business)
   
2. Current year-to-date Profit and Loss statement
3. 1099s or award letters for Retirement, Social Security or Disability benefits income, if applicable

If these apply

• Asset documentation (verification of assets listed on application, i.e. 401K, stocks, bonds, CD's, IRA)
• Copy of first mortgage statement
• Copy of last mortgage statement, to verify current lender name and account number
• Name and phone number of homeowner insurance agent currently using or want to use
• Rental property lease or rental agreement
• Landlord's name, address and phone number to cover last two years
• Copy of present/past bills to pay
• Copy of complete bankruptcy papers
• Copy of complete divorce decree
• Copy of court order for child support, if paying
• Copy of court order for child support, if receiving (past 12 months proof of receipt)
